- Description: What to export, what to clean, how to run old and new systems side by side, and how to plan a go-live that keeps the front desk open.

Changing hospital software is less about the new system and more about the data and the people you bring with you. Most migrations that go badly do so for ordinary reasons: dirty master data, a go-live planned for the busiest day of the week, or a front desk that met the new screens for the first time on launch morning.

This checklist is what we walk through with hospitals before a move. It works whichever system you are moving to.

## 1\. Decide what moves, and what stays behind

Not everything in your old system needs to come across. Agree early, in writing, on three groups:

- **Must move:** patient demographics and IDs (UHID or MRN), active in-patients, open bills and advances, upcoming appointments, current stock with batches and expiry dates, and your price lists and packages.
- **Should move:** past visit history, diagnoses, prescriptions, lab results and discharge summaries, so that doctors can see what happened before.
- **Can stay in the old system:** very old transactions and logs you only need for audits. Keep the old system available read-only for as long as your record-keeping rules require.

## 2\. Clean your master data before you export it

Every duplicate in the old system becomes a duplicate in the new one. The usual suspects:

- **Duplicate patients:** the same person registered twice with a small spelling difference or a new phone number. Merge them first.
- **Phone numbers and addresses:** one format for mobile numbers, and PIN codes in their own field.
- **Doctors and departments:** retired doctors, renamed departments and test entries.
- **Drugs and items:** one name per item, the right HSN code and GST rate, and the same unit across purchases and sales.
- **Tariffs and packages:** remove packages nobody uses and check that every rate is current.

## 3\. Map, then do a trial run

Map each field from the old system to the new one, and list what has no home. Then run a full trial migration into a test copy, not the live system, and have the people who use the data check it:

- the front desk searches for ten real patients by name and phone;
- billing compares totals for a past month;
- pharmacy compares stock on hand for twenty items, batch by batch;
- a doctor opens five patients they know and checks the history makes sense.

Fix what they find and run the trial again. Two or three rounds is normal.

## 4\. Train people on their own work, before go-live

Training works best with each team's real tasks: registration for the front desk, orders and notes for doctors, the dispensing counter for pharmacy, and the daily close for billing. Keep it short and hands-on, and name one go-to person in each department who knows the new system a little better than everyone else.

## 5\. Plan the cutover

- **Pick a quiet day.** Avoid your busiest OPD day and the last days of the month, when billing and stock closing pile up.
- **Freeze the old system** at an agreed time, take the final export, and load it.
- **Carry over what is in flight:** admitted patients, open bills, advances and pending lab orders.
- **Agree a fallback.** Decide in advance what would make you pause, and how the front desk keeps working if something goes wrong.

## 6\. Support the first weeks closely

Have people on the floor for the first days, at the front desk and pharmacy counter especially. Keep a simple list of issues, fix the small ones quickly, and review daily totals against what you expect.

## How this works with Carexa OS

We have importers for the HMS products commonly used in India. Demographics, history and billing records move across, and migration is included in every Hospital and Enterprise onboarding. Every plan includes a 7-day go-live. If you are planning a move, [book a demo](https://carexaos.com/contact?ref=carexaos.com) and tell us what you use today.
