How to choose the best hospital management software in India
Eleven things to check before you sign, from ABDM and claims to offline mode, data residency and how you leave.
Search for the best hospital management software in India and you will find long lists, most of them written by someone selling one of the products on the list. We make Carexa OS, so we are not neutral either. Instead of a ranking, here is the checklist we would use if we were buying: eleven questions that separate software your hospital will live with happily from software it will fight for years.
First, what hospital management software should do
A hospital management system (HMS) runs the hospital day: registration and appointments, the OPD and IPD, orders, lab, radiology, pharmacy, billing and claims, stores and reports. The best ones do all of this on one patient record, so nothing entered at the front desk has to be typed again by the doctor, the lab or the billing counter. If you want the basics first, read what a hospital management system is.
The checklist
1. Is it one record, or modules stitched together?
Ask the vendor to follow one patient from registration to the final bill in the demo. If the lab, pharmacy or billing screens ask for the patient again, the modules are not truly connected, and your staff will do the joining by hand.
2. Is ABDM part of the daily workflow?
ABHA creation and verification at registration, Scan & Share at the counter, linking records to the patient’s ABHA and sharing them on consent should be ordinary screens, not a separate project. See what ABDM is if the terms are new.
3. Does billing match how Indian hospitals get paid?
Look for GST invoices with HSN/SAC codes, packages and tariffs, deposits and refunds, and claims for PMJAY, CGHS, ESIC, ECHS and private TPAs tracked from pre-auth to settlement. UPI collection and daily reconciliation save hours at month end. More in hospital billing software.
4. What happens when the internet goes down?
Outside the metros, connections drop. A cloud HMS that stops when the network does will stop your queue too. Ask whether reception, OPD and billing keep working offline, and how changes sync back. Carexa OS uses an offline-first desktop client for this.
5. Where is patient data stored?
Ask for the hosting region in writing, where backups are kept, and how the system supports the Digital Personal Data Protection Act 2023: consent records, purpose limitation and data-principal requests.
6. Can it move your existing data?
Demographics, history and billing records should come across from your current system, and the migration should be checked with your team before go-live. Ask whether migration costs extra. Our migration checklist covers the details.
7. How is it priced, and what is not included?
Per-seat pricing punishes you for adding doctors and nurses. Per-bed pricing grows with the hospital instead. Either way, ask what is extra: migration, training, updates, interfaces, SMS and WhatsApp, backups. See how Carexa OS is priced for one example.
8. Does it fit your specialties?
A dental clinic needs an odontogram, an eye hospital needs refraction grids, an IVF centre needs cycle tracking. Generic forms slow specialists down. Check for specialty templates, order sets and billing codes.
9. Does it speak the standards?
FHIR R4 and HL7 v2 for records and analysers, DICOM for imaging, LOINC, SNOMED CT and ICD-10 for coding, and an API you can actually use. These decide whether the system can talk to your lab, your PACS and whatever you add next.
10. What does go-live and support look like?
Ask for the rollout plan in days, who trains your staff on site, and what support you get at 2 am. Ask how often updates ship and whether upgrades cost anything.
11. How do you leave?
The question vendors like least. You should be able to take all your data out, in a standard format, without a fee. If the answer is vague, treat it as a warning.
Red flags in a demo
- Slides instead of the live product.
- “That module is coming soon” for something you need on day one.
- Patient details re-entered between departments.
- No clear answer on hosting, backups or data export.
- A price that changes once you mention the number of users.
Take the checklist to every vendor
We keep a longer version of these questions, with our own answers, on Questions to ask any HMS vendor. Use it with every vendor on your shortlist, including us. If you would like to see how Carexa OS answers them on your own workflows, book a 30-minute demo.
